WINTER 2009 Registration Information

PLEASE NOTE: When you hit the "submit" button after filling in your registration form, please make sure you have immediately received an email from us stating that "we have received your registration form but this is not a confirmation you are registered for your class choice" If you have NOT gotten this email your form was NOT RECEIVED by our office and you need to re-submit. If you have any question please call our office at 203-256-1656.
IMPORTANT: The Early Registration deadline date for our Currently enrolled families is Friday, DECEMBER 4th 2008. We will accept ALL forms from our new and returning families PRIOR to DECEMBER 4th, however they will not be entered and processed until AFTER the DECEMBER 4th "current family deadline date so confirmations will not be emailed until after that date". They will be processed on a "first come first served basis". Confirmations will be sent out as we process forms, but NOT until AFTER DECEMBER 4th.

Submission of your form on-line does not mean you are confirmed in your class choices. Your form will be evaluated and once the class choice is decided upon we will e-mail or mail you a letter confirming the class you were assigned!
- Fill out the REGISTRATION FORM completely. We cannot process the form unless you have read the disclaimer at the bottom of this page and indicate that you have read it on the form.
- Please give us as many class choices as possible. This will help us find you a spot in our classes!
- Please take into consideration that young children often have changes in their nap schedules. This may affect your choice of class times!
- If registering on line please include your credit card number and expiration date. We accept ONLY Visa and Mastercard. If you wish you can print this form and fax it to 203-286-1249 or mail it along with your check or money order, payable to Music Together of Fairfield County, Inc.
- Your registration will not be processed without payment. If you are registering on line, but are planning on mailing a check we must receive your check within FIVE (5) days of receiving your registration form, or your registration will NOT BE CONFIRMED and your space may be given away.
- If you are planning to attend a free demonstration class:
- We encourage families to send in registrations before attending a demo, as classes do fill quickly.
- Full tuition reimbursement will be given should you decide to cancel after attending a demo.
- NO REFUNDS/CANCELLATIONS after FRIDAY, DECEMBER 26th 2008! ABSOLUTELY NO EXCEPTIONS!
Families continuing in the same class are ONLY guaranteed their space if their registration form is received before DECEMBER 4th 2008 with the following EXCEPTION:
- PLEASE NOTE: We currently have many families who are attending FULL classes with unregistered siblings (under 8 months old). We cannot guarantee spots for your children in your current class if that unregistered sibling is now over 8 months old and you want to register them along with their previously registered sibling. This is due to restrictions on class size as part of our license with Music Together LLC. We will do the best we can to honor your requests and accomodate the additional children in class, but in the case we are not able to do so, please make sure you provide us with at least one alternate class choice. Please remember we will handle all registrations including Current families registrations on a first come, first serve basis. - All Registrations will be confirmed by either mail and /or email. If you have not received confirmation by DECEMBER 26th 2008. please notify the office.
New / Returning families registrations are handled on a 'first come, first serve' basis.
- IMPORTANT: Families skipping a semester at any time will be registered on a first come/first serve basis, AFTER all currently enrolled families have been placed either in their current class, or the class they are trying to switch into.
Current families: If you need to now switch to another class, different from the one you are currently in, you will NOT BE GUARANTEED YOUR FIRST CHOICE. Please provide us with several class times that work for you.
- Class size is strictly limited.
- A $20 FEE WILL BE CHARGED FOR SWITCHING CLASSES AFTER CONFIRMATIONS HAVE BEEN ISSUED.
- MUSIC TOGETHER DOES NOT PRO-RATE FOR LATE REGISTRATION. Extra Make-ups Will Be Given For Classes Missed.

WINTER 2009 TUITION
All tuition MUST BE PAID IN FULL at time of registration or a space will not be held for your child.
There will be NO REFUNDS, CREDITS OR CANCELLATIONS GIVEN after FRIDAY, DECEMBER 26TH 2008! ABSOLUTELY NO EXCEPTIONS.
REGISTRATION CATEGORIES
CURRENT FAMILY: You are a "Current family" if you took Music Together (MT) in the FALL 2008 semester.
RETURNING FAMILY: You are a "Returning family" if you took Music Together in a semester other than FALL 2008. This includes taking Music Together for any other semester in any US or International location.
NEW FAMILY: You are a "New family" if you have never taken Music Together before.

INDICATES YOUR UNDERSTANDING AND AGREEMENT OF THE FOLLOWING:
Music Together of Fairfield County, Inc. and Jacqueline Jacobs, Director, strive to provide quality music and movement instruction to and for young children and parents and/or caregivers. Music Together of Fairfield County, Inc. and Jacqueline Jacobs cannot protect your children and yourselves from unexpected illness or circumstances due to your health status. We also do not own, maintain, or guard the premises on which instruction takes place. We cannot safeguard your person or property before, during or after instruction. We hope that you will participate mindful to your health, person and surroundings. In recognition of the quality instruction we strive to provide and the fees you have and continue to pay for such services, we ask you to acknowledge that following statements for yourself and your child by signing your name at the bottom of this form.
- A certain level of fitness may be required for participation in instruction. Each student, by and through his or her parent(s), each parent and each caregiver certify that each is in good health and has no known medical condition that could endanger their own well being of others in the program. Each certifies that she/they have consulted with a physician and determined that participation does not jeopardize his/her/their health.
- The premises, at which instruction is provided is not owned, inspected, maintained, or controlled by Music Together of Fairfield County Inc. and/or Jacqueline Jacobs. Each child, parent and caregiver hereby release, and for him/herself and on behalf of the student if he/she is a minor, waive and discharge Music Together of Fairfield County, Inc. and Jacqueline Jacobs, individually and their respective employees, agents, assignees and other related entities from any and all liability for any and all loss or damage and any claim for loss or damages arising from the participation or attendance at this premise, to property or per person, even injury or loss or claim which arises out of the condition of the premises, whether such condition is know or unknown. Each participant inspected the location for instruction and is satisfied with the premises condition.
- Each child, parent and caregiver or other participant in consideration for participation in the instruction provided by Music Together of Fairfield County, Inc. and Jacqueline Jacobs, individually hereby releases, and for him/herself and on behalf of the student if he/she is a minor, waives and discharges Music Together of Fairfield County, Inc. and Jacqueline Jacobs, individually and their respective employees, agents, assignees and other related entities, from any and all liability for and all loss or damage to property or to person and any claim which could not have been anticipated at the time of enrollment or any and all future MT Classes or events attended.
